Place I used to work at had an old e150 van. A bunch of us had complained that it was a piece of shit and needed to be retired and it wasn't safe to drive anymore. The higher ups snubbed their noses at us. One afternoon me and a coworker were driving it to a jobsite on I294 at 65 mph and the drive shaft came loose. Went right thru another car's windshield. Nobody was hurt. But man did we get yelled at. "Why didn't you say something about the van not being safe?" "Why did you drive it if you didn't think it was safe?" So now it was our fault. I guess it was a good lesson for me later in my career because I am very anal about maintenance and safety now. I take zero chances.
